Understanding the Pelvic Floor

The pelvic floor is a group of muscles located within the pelvis that provide support to the lower organs, including the bladder, bowel, and reproductive structures. These muscles play an essential role in bowel and bladder control, sexual function, and overall core stability.

Who Should Consider a Pelvic Floor Assessment?

A pelvic floor physiotherapy assessment may be beneficial for individuals experiencing concerns related to bladder and bowel function, pelvic pain, organ prolapse, or sexual health. Common conditions treated include pregnancy and postpartum recovery, pain during intercourse, urinary or fecal incontinence, tailbone pain, and pelvic organ prolapse.

What to Expect at Your First Pelvic Floor Physiotherapy Appointment

Initial assessments typically last about 60 minutes. During this time, your physiotherapist will discuss your medical history, lifestyle, and any symptoms you are experiencing. This may include questions about bladder and bowel habits, sexual health, pain levels, and functional goals. A physical evaluation of posture, lower back, pelvis, and hip movement may be conducted. The physiotherapist will also explain pelvic floor anatomy and its role in your condition. If appropriate, and with your consent, an internal examination may be suggested to better understand muscle function and coordination. Abdominal function might also be assessed. Following the assessment, your physiotherapist will explain the findings and develop a personalized treatment plan with recommendations for improving your condition.

What is an Internal Assessment?

An internal assessment allows the physiotherapist to directly evaluate the function of the pelvic floor muscles through the vaginal or rectal route, without the use of instruments. With your consent, the therapist will guide you through the process, ensuring comfort and addressing any questions or concerns. This assessment helps to evaluate muscle strength, endurance, relaxation, and areas of tension, allowing for targeted treatment recommendations tailored to your needs.

What if I’m Not Comfortable with an Internal Assessment?

While an internal assessment provides the most detailed information, it is not mandatory. If you prefer, an external evaluation can be performed instead, though it may offer less precise insights. Some individuals opt to use the first visit as an opportunity to discuss concerns and learn about the pelvic floor before undergoing a physical examination.

Pelvic Floor Physiotherapy During and After Pregnancy

It is beneficial to begin pelvic floor physiotherapy early in pregnancy (from 6 weeks onward) to establish a care plan. A session around 35 weeks can help prepare the pelvic floor for labor and delivery. Postpartum assessments are recommended at around six weeks after birth to support recovery, address any symptoms, and discuss safe return to physical activity.

Are There Any Reasons to Avoid an Internal Assessment?

An internal pelvic floor examination is not recommended if you have an active infection, such as a urinary tract infection. Your physiotherapist will discuss any contraindications and adjust the assessment accordingly.

What if I Have Pelvic Pain and Am Unsure About an Exam?

If you experience pelvic pain and are uncertain about an internal examination, you are still encouraged to seek pelvic floor physiotherapy. Your therapist will create a safe and supportive environment, working with you to find a comfortable starting point that respects your boundaries.

Can an Assessment Be Done While Menstruating?

Yes, a pelvic floor assessment, including an internal exam, can still be conducted during menstruation if you are comfortable with it.

Muscle testing and assessment of sports injuries and a suite of musculoskeletal conditions found in all age groups. Early in life Kirsten realized a career in healthcare was what she would excel at. Coming from a family with a background in Medicine and Engineering she found the combination of academics and science, and the hands on skills of Registered Massage training suited her very well.

Kirsten graduated from the West Coast College of Massage Therapy in December 2012. Since graduation Kirsten has provided Massage Therapy for the elderly, athletes, professionals, and clients with rehabilitation needs.

Previous to being an massage therapist, she completed 5 years in the military which inspired her to enter into a field which assisted those who are injured, or in need of care. Scott completed a post-doctoral fellowship (FCAMPT) in Physiotherapy at University of Western Ontario, having previously completed Doctorate and Masters degrees. He enjoys applying the most up to date evidence based treatments to help patients get objectively based results better in fewer visits using Manual therapy, education and therapeutic exercise.

He is proud to serve our local community and is committed to providing one-on-one care with integrity, innovation and energy. He teaches in both the Canadian and American Orthopedic Fellowship program and actively mentors other therapists in their pursuit of excellence.

Scott completed his professional Physiotherapy degree at Dalhousie University in Halifax, Nova Scotia after doing a degree in Kinesiology. He was an Academic and Athletic All-Canadian during this time. In 2000, he was awarded a yearlong Scholarship at the Australian Institute of Sport, working with world-class scientists and athletes.

He has continued to develop his knowledge over the years by taking a multitude of advanced orthopedic and sports related courses. He has worked with numerous elite sports disciplines including triathlon, rowing, soccer and athletics. He has been honoured to provide his professional skills internationally as Team Physiotherapist for the Canadian National Team.

Scott has been an avid athlete from a young age and understands the impact and benefits that activity can have on the body. He has participated in soccer, golf, hockey and baseball at a provincial level, but has excelled in endurance sports, notably distance running. After graduating, Ivy trained full time for two years as a part of the senior Canadian national team Olympic camp for rowing before heading to Montreal to complete her masters of physiotherapy from McGill.

In her daily life, Ivy cherishes spending quality time with friends, cooking and finding tranquility through exercise. Ivy’s commitment to daily movement is reflected in her enthusiasm for trail running, cycling, racket sports, and weightlifting.

Lauren’s interest in kinesiology stemmed from her background in competitive ballet, tap, and modern dance. During her time dancing, Lauren starred in several productions of The Nutcracker and, alongside her teammates at the Coast Academy of Dance, won first place in numerous dance competitions. Additionally, she completed advanced ISTD dance examinations and underwent two years of ISTD dance instructor training.

As a kinesiologist, Lauren integrates her knowledge of movement from her experience as a dancer with evidence-based kinesiology practices. Iris graduated with her Masters of Physiotherapy from Queens University after completing an Honours Bachelor of Science in Kinesiology from the University of Victoria. Between Victoria, Ottawa and Calgary, she has worked as a physiotherapist in various settings including sports, orthopaedics and neuro-rehabilitation. As a lifelong learner, Iris does ongoing professional development, and has training for complex musculoskeletal injuries, dry needling, vestibular rehabilitation and is a fellow of the Canadian Manual and Musculoskeletal Physiotherapy (FCAMPT). In addition, Iris has taken a variety of courses targeting orthopaedic and neurological conditions, and has a special interest in shoulder rehabilitation and running-related injuries.
